Other requirements (Needs an appropriate header)

  1. Attested photocopies of parents' passports. If neither parent has a passport, affidavits from both parents (specimen at Annexure 'D' of the passport form). If only one parent has a passport, an attested photocopy of the passport and an affidavit from the other parent (specimen at Annexure 'D' of the passport form)
  2. Column 19 of the application form to be signed by both parents/ legal guardian
  3. The minor's signature or thumb impression in Column 22
  4. In case one parent is abroad, an affidavit from them providing a No Objection certificate to the issue of passport to the minor, counter-assigned by the Indian Mission/Consulate abroad
